Health Office - Health Appraisals - May Physicals
We highly recommend each student to see their primary physician for an annual physical. School physicals are offered as an option for any student needing a health appraisal.
AuSable Forks Elementary: May 11
Completed Health Appraisal forms must be returned to the Primary Health Office by May 5.
Keeseville Elementary: May 18
Completed Health Appraisal forms must be returned to the Elementary Health Office by May 5.
Middle High School: May 4 and May 18
Completed Health Appraisal forms must be returned to the MHS Health Office by Thursday, April 29.
Health Appraisal Forms may be picked up at the Health Offices or down loaded here.
